The role prioritizes and coordinates discharge planning through <strong>critical thinking, teamwork, and effective communication<\/strong> with providers, patients, families, and external vendors to support <strong>timely, safe discharge<\/strong> and appropriate post-discharge care.<\/p>\n<p>Management reserves the right to revise the job description or assign additional responsibilities based on operational needs.<\/p>\n<hr>\n<\/hr>\n<h3><strong>Key Job Accountabilities<\/strong><\/h3>\n<p><strong>Collaboration with Care Coordination Team<\/strong><\/p>\n<ul>\n<li>Partner with\u00a0<strong>Case Managers and Social Workers<\/strong>\u00a0to:\n<ul>\n<li>Assess baseline patient needs<\/li>\n<li>Identify post-hospital support requirements<\/li>\n<\/ul>\n<\/li>\n<li>Support development and execution of the\u00a0<strong>TOC plan<\/strong>, including:\n<ul>\n<li>Communicating provider options and available resources<\/li>\n<li>Clarifying expected discharge timelines<\/li>\n<\/ul>\n<\/li>\n<li>Ensure patients and facilities understand\u00a0<strong>benefit coverage<\/strong>; coordinate with payers as needed<\/li>\n<li>Monitor progress toward TOC goals and\u00a0<strong>escalate barriers<\/strong>\u00a0to Case Managers and Social Workers<\/li>\n<li>Keep patients and facilities informed of TOC plan progression<\/li>\n<li>Coordinate\u00a0<strong>post-discharge referrals and authorizations<\/strong><\/li>\n<li>Communicate referral status and barriers throughout the day to support timely discharge<\/li>\n<li>Facilitate\u00a0<strong>patient transfer to post-acute facilities<\/strong>, including:\n<ul>\n<li>Preparing documents<\/li>\n<li>Obtaining physician signatures<\/li>\n<li>Coordinating transportation<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong>Departmental Goals &#038; Objectives<\/strong><\/p>\n<ul>\n<li>Round with Case Managers and Social Workers to provide updates and receive direction<\/li>\n<li>Deliver\u00a0<strong>Medicare \u201cImportant Message\u201d (IM)<\/strong>\u00a0and educate patients\/facilities on appeal rights<\/li>\n<li>Proactively identify and escalate\u00a0<strong>barriers to discharge<\/strong><\/li>\n<li>Participate in\u00a0<strong>daily huddles and department meetings<\/strong><\/li>\n<li>Contribute to\u00a0<strong>team prioritization and problem-solving<\/strong><\/li>\n<li>Collaborate on interdependent tasks and demonstrate flexibility<\/li>\n<li>Share knowledge and foster\u00a0<strong>collaborative relationships<\/strong><\/li>\n<\/ul>\n<p><strong>Communication &#038; Documentation<\/strong><\/p>\n<ul>\n<li>Maintain accurate and timely documentation\u00a0<strong>per department standards<\/strong><\/li>\n<li>Enter Care Coordination notes in the\u00a0<strong>EMR<\/strong>\u00a0(Epic;
