Care Coordinators (Case Managers)
Northeast Ohio and North Central Ohio
Hybrid role (Mobile: telephonic and in-home/community visits)
3-month contract with potential for extension or full-time employment

ABOUT THE ROLE

Our client, a leading payer in the Ohio market, is seeking 20 Care Coordinators (Case Managers) to support the MyCare Community Well Care Management functions in Northeast and North Central Ohio. This expansion is driven by rapid and higher-than-anticipated membership growth within the OH Duals Program, requiring increased care management capacity. The role is mobile, involving a mix of telephonic and in-home/community visits within geo-assigned territories to minimize travel. Care Coordinators will support regions going live on 1/1, with start dates of 1/5 or 1/12 depending on training class availability. This is a 3-month contract position with the possibility of extension or transition to full-time employment.

WHAT YOU’LL DO

  • Conduct Health Risk Assessments (HRA) for assigned members via telephonic and in-person visits
  • Develop, implement, and monitor individualized care plans based on member needs
  • Coordinate community benefits and services to support member well-being
  • Collaborate with provider networks and community resources to ensure continuity of care
  • Manage a caseload of up to 250 primarily low-risk members, including both engaged and unengaged populations
  • Document all member interactions and care activities in compliance with organizational and regulatory standards
  • Travel within geo-assigned territories for in-home and community visits as required
  • Participate in training sessions and ongoing professional development as needed

WHAT YOU BRING

  • Experience in home health, hospice, case management, provider-based coordination, or similar community-based roles
  • Relevant clinical licensure (e.g., Social Worker, Licensed Professional Counselor, Registered Nurse)
  • Ability to conduct HRA assessments and complete care plans
  • Comfortable with a mobile role involving telephonic and in-home/community visits, including entering member homes and working within the community
  • Strong collaboration skills with provider networks and community resources
  • Ability to manage caseloads of up to 250 members, including both engaged and unengaged populations
  • Willingness to travel within assigned geographic territories in Northeast and North Central Ohio
  • Openness to diverse clinical backgrounds and a commitment to supporting vulnerable populations
