Nurse Navigator – Community-Based Disease Intervention Specialist (CBDIS)

4-month contract

Start Date: 10/31

40 hrs/week

100% On-Site in Washington DC (With at home visits as needed)

Responsibilities:

-Guide patients through diagnostic tests, treatments, and follow-up care

-Helping patients navigate the medical system, understand their diagnosis and treatment options, and address logistical or emotional barriers to care (e.g., appointment scheduling, insurance issues, emotional support)

-Following up with patients on cooperation with their treatment plans, whether or not they are taking medication, etc.

-Follow up on appointments, whether at the hospital or elsewhere

-Data collection

-Identifying/documenting variables as to why patients aren’t taking meds, keeping them accountable for their treatment

-Will involve at-home patient visits as needed

-No clinical work, no test administration, no vitals, etc.

Qualifications (required):

-RN certification

-Bachelor’s Degree

-2-3+ years of experience as a CBDIS specialist/nurse navigator supporting STI research

-Experience/specialization in HIV/STIs (Gonorrhea, Chlamydia, HIV, & especially Syphilis)

-Knowledge of, and experience with, the Ryan White Program, HIV, STIs

-Must be open-minded and possess strong emotional intelligence

Qualifications (preferred):

-Possess knowledge of the Washington, DC area and DC programs/resources available to patients (local clinics, other care sites, insurance /coverage resources, etc.)
